Liability Agreement: In consideration of being permitted by Laurel Street Arts to participate in an in-studio class, I hereby waive, release and discharge any and all claims for damages or personal injury, death, or property damage which I may have or which may hereafter accrue as a result of my participation in said activity. This release is intended to discharge, in advance, Laurel Street Arts, its officers, directors, members and managers, and all other members of its studio staff whether contracted or employed, from and against any and all liability arising out of or connected to in any way with any participation in said activity. I understand that the activity that I am participating in may be of a hazardous nature and/or include physical and/or strenuous activity. Knowing the risks involved, nevertheless, I have voluntarily applied to participate in said activity, and I hereby agree to assume any and all risks of injury or death and to release and hold harmless Laurel Street Arts, its officers, directors, members and managers, and all other members of its studio staff whether contracted or employed, who might otherwise be liable to me.

Illness/behavior: I waive all rights to any legal action against Laurel Street Arts in the event of injury, illness, damages, loss of personal property not caused by or resulting from the negligence of the owner or attendees in the studio. Preventative measures will be practiced to reduce the spread of COVID-19 and other illness by refusing admission to anyone appearing to not be feeling well. I understand that Laurel Street Arts reserves the right to dismiss anyone from this class for reasons such as, but not limited to, signs of illness, disruptive or unsafe behavior, behavior unsuitable for the comfort of the classroom environment, or poor performance at the sole discretion of Laurel Street Arts.

Photo Release: I give permission for my photo to be taken during in-studio classes. I understand that the photos taken at Laurel Street Arts are for the sole purpose of documenting the various experiences offered and may be used on Laurel Street Arts social media and website.
